About

The Sublette Center was founded as a 501c3 nonprofit in 1978 by a committed group of local visionaries, including local ranch families, medical professionals, entrepreneurs, and philanthropists. Prior to 1978, elderly residents of Sublette County had to leave the area to access long-term care or to find independent living services.

In 2023 we merged with the Sublette County Hospital District. And in 2025 we moved into our new home, the Heritage Home, at 550 Fremont Lake Rd.

We are currently part of the Sublette County Hospital District led by a five member elected board who are passionate about providing quality living for our seniors.

Pinedale Wyoming

Pinedale is surrounded by the stunning Wind River, Wyoming, and Gros Ventre mountain ranges. It is perfectly located amidst the greater Yellowstone ecosystem in the Bridger-Teton National Forest. This amazing backdrop provides our residents with unique opportunities to enjoy the beauty of this area, wildlife, and fresh air while receiving loving care and services.

The Heritage Apartments and Aspen Grove apartments are located within walking distance to many local gems including the Pinedale Auditorium (a favorite venue for many Pinedale Fine Arts Council productions), Rendezvous Pointe (our senior community center), and the Pinedale Aquatic Center. Groceries, hair salons, banking, dental services, physical therapy and more are all nearby.
