Summary Description:
While upholding the Sublette County Hospital Districts mission, vision and values, the Chief Nursing Officer directs, administers, and coordinates the activities of the hospital, clinics, and outpatient nursing services in support of policies, goals and objectives established by the Chief Executive Officer and the Governing Body. Must be prepared to handle an evolving integrated health care delivery system, technological innovations, an increasingly complex regulatory environment and restructuring of work with focus on the SCH’s mission and vision for patient centered care. Assists in the development and implementation of SCH’s strategic plan and ensures standards set are implemented and maintained. Develops and maintains collaborative relationships with local and regional health care partners and affiliates. Serves as a visible and enthusiastic champion in providing day to day leadership and guidance to the District nursing staff to ensure service excellence.
Supervision Received:The Chief Nursing Officer reports directly to the Chief Executive Officer.
Responsibilities:
Organizes programs, policies and procedures are developed to assess, evaluate, and meet the needs of the patient:
1. Responsible for the provision of nursing services 24 hours a day, seven (7) days a week.
2. Implements standards of nursing practice reviewing and revising as necessary to reflect changes in nursing practice and patient population.
3. Ensures that clear, concise, and current written policies and procedures are available to assist the staff and minimize risk factors. Nursing policies are approved by the Nurse Executive before implementation
4. Plans and coordinates with the Chief Financial Officer, utilizing the respective service managers for planning the budgeting requirements for staff, performance of work, supplies, and equipment.
5. Demonstrates a professional commitment by initiating, developing, and maintaining professional training and continuing education programs and cooperative relationships with the community, state, and other institutes of learning.
6. Recommends modifications, additions, or deletions to nursing staffing plans to ensure reasonable hours and acceptable working conditions to provide optimal patient care coverage.
7. Initiates and participates in problem solving policy forming conferences for patient care services. Maintains close coordination with all departments to ensure continuity and collaboration of services.
8. Ensures that all aspects of patient care are carried out in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
9. Represents the nursing staff and the mission of the SCHD by active involvement in delegated committees and projects.
Participates with the Governing Body, management, medical staff, and clinical leaders in the hospital’s decision-making process:
1. Plans and recommends to Chief Executive Officer new facilities or equipment or modifications needed to provide optimal patient care.
2. Serves as a member of professional staff committees in matters pertaining to patient care and participates in the Executive, Finance, Governing Body, and medical staff departmental meetings.
3. Provides suggestions for cost savings and/or revenue opportunities and is responsible for cost controls to ensure maximum effectiveness of funds expended from approved departmental budgets.
4. Collaborates with hospital leaders in developing, implementing, reviewing, revising, and monitoring hospital wide performance improvement activities.
Demonstrates Competency in the following areas:
1. Implements an effective, ongoing program to measure, assess and improve the nursing care delivered to patients.
2. Responsible for developing and/or maintaining policies, procedures, and controls to promote communication and adequate information flow that ensures consistent quality and safety.
3. Establishes operating policies that are consistent with the Chief Executive Officer’s broad policies and objectives to ensure their execution.
4. Regularly and systematically monitors utilization of services adjusting where necessary to meet patient needs and provider/employee satisfaction and communicates these results to the CEO.
5. Assists hospital team with established quality indicators and works with specific departments to implement improvements/changes to achieve quality levels.
6. Assures that nursing leaders review monthly performance improvement compliance with established indicators and sees that corrective actions are taken when negative outcomes are identified.
7. Responsible for the planning and implementation of new programs and/or new equipment to assure an optimal level of safety and quality. Delegates implementation as appropriate.
8. Responsible for providing direction and feedback on patient flow and engagement to assure optimal satisfaction with visit.
9. Collaborates with departmental leadership regarding appropriate staffing levels and interventions to facilitate effective delivery of patient care and optimal customer satisfaction.
10. Participates and assists in planning and providing of Inservice education programs within the hospital or other meetings, seminars, and institutes conducted outside the hospital
11. Follows Pharmacy protocols for obtaining drugs when the Pharmacist in unavailable.
12. Regularly schedules departmental meetings and assigns appropriate departmental representatives to the multi-disciplinary committees.
13. Ensures compliance with policies and procedures regarding department operations, fire safety, emergency management, and infection prevention and control.
14. Maintains regulatory requirements, including all federal, state, local regulations, and accrediting organization standards.
Qualifications:
1. Graduate from an accredited school of nursing and holds current unemcumbered licensure in the State of Wyoming as a registered professional nurse.
2. Bachelor’s degree in nursing is required.
3. Master’s degree in nursing or equivalent is preferred.
4. Minimum of 6 years’ experience as a staff nurse and nurse manager.
Knowledge, Skills, and Abilities:
1. Ability to effectively present written and verbal information to top management, medical staff, public groups, and Governing body.
2. Ability to read, analyze and interpret financial reports and contracts.
3. Ability to prioritize and complete multiple tasks in a timely manner.

Summary Description:
While upholding the Sublette County Hospital Districts mission, vision and values, the Skilled Nursing Facility the Registered Nurse coordinated and delivers, on an age and developmentally appropriate basis, direct clinical care per nursing process for adult and geriatric residents/patients. The RN shall utilize knowledge of residents’ rights, legal/ethical concepts, communication skills, safety and infection prevention while performing the following: basic nursing skills, basic restorative skills, mental health and psychosocial skills, and nursing team member skills.
Supervision Received: Directly supervised by the Director of Nursing.
Responsibilities:
Administrative:
Provides direct supervision and coordination of the resident care team. Is responsible for the quality of care given by self and the resident care team.
Participates in patient and resident care through procedures within the scope of their training and practice, according to state and federal regulations and facility policies and procedures.
Assures that quality of care is given to each resident by supervising, directing, and motivating the CNA staff in their charge according to the plan of care and policies and procedures.
Reviews CNA’s resident assignments and duties at the beginning of the shift and performs ongoing evaluation of residents needs and communicates to the CNAs throughout the shift.
Provides feedback to the DON regarding performance of nursing assistants or other care staff.
Communicates staffing problems and needs to the Director of Nursing.
Supports and maintains a culture of safety and quality.
Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors.
Meets with members of the interdisciplinary team for development, implementation, and evaluation of residents’ care plans and protects resident from neglect and abuse.
Promotes residents rights, provides interventions to increase residents’ quality of life and self-worth and involves resident in decisions affecting their care.
Consistently involves resident and family in planning for care, treatment, teaching needs, and discharge plans. Initiates and leads resident care conferences and updates plan of care, as appropriate.
Functions as a leader for the CNAs and LPNs/LVNs by delegating in a manner that contributes to excellence if the nursing unit.
Clinical:
Assesses residents on admission ensuring that the nursing assessment is completed within 24 hours and all other assessments are completed within 72 hours. Submitting all documentation in a timely, clear, concise, and pertinent manner.
Treats residents and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of residents and their families.
Provides direct patient care, according to physician’s orders and individual resident’s needs, including administration of medications, treatments, therapies and performs and assist with ADL’s, in accordance with recognized standards of practice and organizational standards.
Identifies and reports changes in condition to the attending or on call physician, the DON, and the responsible family member. To include documentation of this change in the progress notes for the resident’s medical record.
Ability to assess and reassess pain. Utilizes appropriate pain management techniques. Educates the resident and family regarding pain management.
Assesses residents and completes monthly nursing summary as well as the quarterly assessment and documents appropriately including Minimum Data Sets.
Notes, carries out or ensures that physician’s orders are performed in a timely manner.
Demonstrates ability to interpret results of waived tests; takes appropriate action on waived test results or any abnormal lab or radiology results.
Follows proper infection prevention and control procedures and supervises supportive staff in infection prevention and control techniques.
Understands the policies on restraints their proper use.
Actively receives change of shift report and knows location of assigned residents.
Gives a thorough report to the oncoming shift to ensure the unit is left in good condition and oncoming shift knows assigned residents and their status.
Follows through, as needed, on information given during shift report, by resident, or by family regarding resident concerns.
Provides all the above general activities to residents in isolation.
Adheres to all policy and procedures of Sublette County Health and the Nursing Department.
Attends staff development activities to improve knowledge and skills and to maintain competency and certification.
Maintains personal health and wellness to prevent transmission of illness to patients or residents.
Other duties as assigned by Director of Nursing of SCH
The job holder must demonstrate current competencies applicable to job position.
Qualifications:
Graduate of an accredited school of Nursing.
Current unrestricted license as a Registered Nurse in the State of Wyoming.
Minimum of one-year experience in geriatric care preferred.
Current certification in Basic Life Support or certification within 3 months of hire.
Knowledge, Skills, and Abilities:
Possesses a genuine interest and concern for adults in later years of life.
Demonstrates excellent verbal and written communication skills.
Knowledge of long-term care procedures and goals.

Working Conditions:

Frequent contact with a variety of people including prisoners and people with mental, emotional, and physical disabilities.
Medical office, exam room, laboratory, radiology, emergency room and professional practice settings. Frequent exposure to communicable diseases and other conditions common to rural clinic environments.
May require travel between District sites.
Physical Demands:
Varied activities including moving, reaching, bending, and lifting. Requires full range of body motion including handling, moving, positioning, and transporting patients. Also requires manual and finger dexterity, and eye-hand coordination. Requires standing and moving for extensive periods of time. Occasionally moves items weighing up to 50 pounds without assistance. Requires corrected vision and hearing to normal range. This position frequently requires working under stressful conditions and irregular hours. Year-round highway travel between clinics and other facilities throughout the district is required.

ER Physician
The Sublette County Hospital District is currently seeking a physician for the emergency department Pinedale Medical Clinic and future Sublette County Hospital Emergency Room. The current emergency department offers 24/7 emergency services at the Pinedale Clinic in Pinedale, Wy and this position would transition to the Sublette County Hospital upon completion, expected summer 2025. This physician would work eight (8) 24-hour shifts per month. The best candidate for this position would be experienced in emergency medicine and comfortable providing solo coverage with limited support. PRN opportunities are also available. This is truly a rural clinic as the nearest hospital is approximately 78 miles away.

The Sublette County Hospital District is currently seeking a full-time board-certified Family Practice Physician for the Pinedale and Marbleton, Wyoming medical clinics. The clinic is open Monday - Friday, 8-5 and has an attached emergency department offering 24 hour a day emergency services. The best candidate for this position must be willing to work in a clinic setting, seeing an average of 13 - 15 patients per day. This is truly a rural clinic as the nearest hospital is approximately 78 miles away.
The position offered would include 5 days per week in the clinic. Summary Description:
Responsible for providing professional health care to clinic patients following established standards and practices of care. Assists the health care team in coordinating patient care.
Supervision Received: Directly supervised by the Director of Nursing.

Summary Description:
MRI Technologist are responsible for performing MRI procedures and producing diagnostic quality images at a technical level not requiring constant supervision. MRI Technologists will perform a variety of technical procedures that require independent judgment, ingenuity, and initiative in applying prescribed radio waves for radiologic diagnosis and assumes responsibility for designated area or procedures as required or limited by licensure.
Supervision Received: Directly supervised by the Director of Radiology
Responsibilities:
1. Performs a variety of specialized tasks involved in the performance of MRI scans to aid in the diagnosis of patient conditions in accordance with departmental protocols.
2. Perform complex specialized tasks associated with the operation of MRI scanners and related equipment in accordance with prescribed safety procedures. Screens patient appropriately to ensure safety and monitor all items brought into the MRI room.
3. Complete a medically acceptable exam in a minimum amount of time while producing the maximum amount of information. Optimize computer images to enhance diagnostic information for Radiologist interpretation.
4. Effectively communicate preparation instructions to a variety of audiences (patients, providers, and other medical staff)
5. Properly identify patients and exam being performed.
6. Thoroughly explains the procedure in an empathetic and caring way to alleviate patient anxiety. Assures patient’s questions are answered prior to beginning exam.
7. Accurately position patients for MRI scans using immobilization and protective equipment as necessary. Display capability to modify positioning and technique in order to provide the radiologist/provider with special images per their request.
8. Ability to start IV’s, load and operate power injectors for contrast media administration and inject contrast.
9. Operate appropriate MRI equipment safely and proficiently.
10. Demonstrates a thorough knowledge of cross-sectional and sagittal anatomy.
11. Utilize appropriate technical factors to obtain quality images.
12. Demonstrate a thorough understanding of all required paperwork, including all requisitions, consent forms, logs, schedules, patient charts; PACS image archival, time sheet, forms, etc.
13. Evaluate MRI scans for technical quality, collate processed images in sequence and properly mark all images with correct identifiers including patient name, birthday, MRN and any other patient identifier as used by the district.
14. Demonstrate proper body mechanics when moving/ lifting patients and supplies to ensure employee and patient safety
15. Understand that their position does not allow them to offer an opinion to the patient or a family member regarding a radiologic study. However, occasional discussion with the provider regarding a professional opinion is permitted only in emergency situations.
16. Provide a copy of current licensure to the Director of Radiology and to Human Resources.
17. Obtain continuing education credits required to uphold licensure and maintain certification in CPR.
18. Provide orientation and training to new MRI technologists and provide education and training to student technologist.

Summary Description:
Collection and processing of samples, performing routine laboratory testing, performing and documenting quality control, documenting patient results and reporting results according to laboratory policy. These duties will include specific testing applicable only to Medical Technologists and regular oversight of non-certified laboratory staff.
Supervision Received: The Medical Technologist will report directly to the Laboratory Director
Responsibilities:
1. Orders or retrieves orders in the Electronic Medical Record and processes the orders in the LIS.
2. Ensures that paper orders and electronic orders are complete and if not; obtains a complete order.
3. Follows the laboratory’s procedures for specimen collection, handling and processing.
4. Adheres to the laboratory’s quality control policies by performing and documenting all QC activities and all corrective actions.
5. Follows the laboratory’s policies whenever test systems are not within the laboratory’s established acceptable levels of performance.
6. Identifies problems that may adversely affect the performance or reporting of test results and either correcting the problem or notifying the appropriate supervisor.
7. Performs patient testing on all analyzers and kits. Follows the lab protocol regarding abnormal results and approves results if they are appropriate.
8. Performs testing on proficiency testing samples and maintaining required records per laboratory policy. Maintains records which demonstrate that proficiency testing samples are tested in the same manner as patient specimens.
9. Performs Manual Differentials.
10. Performs Urine Microscopic exams.
11. Retrieves results if requested to do so.
12. Maintains inventory of laboratory supplies and places orders, following inventory control protocol to ensure all ordered products are received and labels all reagents with received date and in-use date and maintains reagent logs.
13. Handles laboratory related calls and all issues involving the reference laboratory.
14. Adheres to District billing practices for laboratory services.
15. Follows and documents instrument maintenance programs for laboratory equipment, performs calibrations on the Axcel, the Stratus and the Tosoh per laboratory procedure, performs verification on the Axcel, the Stratus, and the Tosoh semi-annually.
16. Validates new instrumentation.
17. Oversees QC and maintenance programs, monitoring the Axcel, Sysmex, Stratus and Tosoh QC results for shifts and trends.
18. Ensures that analyzers and individual kits are always ready for after-hours use.
19. Performs duties as assigned by the Laboratory Director.
20. Performs Training and competency requirements as assigned in a timely manner.
21. Maintains QAP data and ensures that the data is submitted monthly and the report printed monthly.
Qualifications:
1. MT (ASCP) certification or equivalent.

1. Knowledge of CLIA and COLA regulations governing the laboratory.
2. Knowledge of Compliance program guidelines.
3. Knowledge of common safety hazards and precautions to establish/maintain a safe work environment.
4. Knowledge of CPT and ICD-9/10 coding.
5. Skill to investigate problems, organize information and present a collaborative solution.
6. Skill to identify potential problems and provide resolutions.
7. Ability to train and educate other providers and staff.

Working Conditions:

4. May require travel between District sites.

Varied activities including moving, reaching, bending, and lifting. Requires full range of body motion including handling, moving, positioning and transporting patients. Also requires manual and finger dexterity, and eye-hand coordination. Requires standing and moving for extensive periods of time. Occasionally moves items weighing up to 50 pounds without assistance. Requires corrected vision and hearing to normal range. This position frequently requires working under stressful conditions and irregular hours. Year-round highway travel between clinics and other facilities throughout the district is required.

Summary Description:

Responsibilities:
Answers telephone, screens call, takes messages and provides information.
Provides directions, information, and other assistance as needed.
Must maintain the strictest confidentiality.
Updates patient information, collects payments, and provides receipts.
Maintains and updates current information on physicians’ schedules ensuring that patients are scheduled properly, and appointments are confirmed.
Performs various clerical duties such as photocopying forms, reports, and patient information, typing letters, forms, labels, mailing and faxing forms.
Performs other business office functions as directed by their manager.
Qualifications:
High school graduate required.
Ideally, two (2) years of work experience preferably in a medical office setting.
Knowledge of medical terminology desirable.
Word processing and computer experience preferred.

Summary Description:
The Certified Medical Coding Specialist is primarily responsible for translating clinical and/or ambulance work into code and the data entry of the appropriate billing codes into the practice management system for reimbursement from the appropriate party. The Coding & Billing Specialist provides internal chart compliance audits and assists with providing coding & billing education to the providers, clinical staff and ambulance crews, as necessary.
Supervision Received: The Certified Medical Coding Specialist reports directly to the Business Office Manager.
Responsibilities:
Apply appropriate and accurate E&M, ICD-10, CPT and HCPCS Codes and modifiers from medical record documentation to ensure correct claim submission.
Resolves medical record documentation deficiencies through healthcare provider query, provider education and provides routine feedback to healthcare providers to correct deficiencies.
Performs quality assessment of records, including verification of medical record documentation, both electronic and written and works with QA team to improve quality of documentation.
Responsible for researching errors and missing documentation from medical record, in order to provide the most accurate coding process.
Assist with correcting and resubmitting insurance denied claims. Identifies need for corrective action to ensure errors are not recurring.
Maintains up to date credentials.
Maintain and participate in coding and billing continual education opportunities.
Maintain good communication with outside affiliations (Sublette Center, Lincare, etc.).
Identify and correct practice management system errors, omissions and inconsistencies.
Answer the billing phone and assist guarantors or patients as they call in with questions, comments or concerns.
Update patient demographic information within the practice management system when necessary.
Cross trains with other business office personnel.
Performs other business office functions as directed by the Business Office Manager.
Attend all District staff and team meetings as requested. Conducts special projects and studies as directed.
Qualifications:
High school graduate required. Certified Coding Certificate required. Some College helpful.
Ideally, two (2) years experience using a medical software system.

Knowledge of the CPT, HCPCS and ICD coding parameters.
Knowledge of Anatomy and Physiology.
Knowledge of medical terminology.
Knowledge of current health care compliance regulations.
