As a patient, you have the right under Federal law to file a complaint or grievance regarding the quality of medical care you received during your visit, or concerns regarding a premature discharge. You have the option to file your complaint on the contact page at our website at OR you may call either clinic to voice your concerns. You will need to provide your name, the patient’s name (if you are submitting a complaint on behalf of the patient), the date the incident occurred, the staff involved, details about the incident, and your contact information.
