Care coordination involves deliberately organizing patient care activities and sharing information among all the participants concerned with a patient's care to achieve safer and more effective care. This means that the patient's needs and preferences are known ahead of time and communicated at the right time to the right people, and that this information is used to provide safe, appropriate, and effective care to the patient.
There is no cost to enroll in Care Coordination.

Cara Merry, RN - Care Coordination Manager
Born in Florida, raised in Texas, then moved to South Carolina at age 12. Working in healthcare since 2004, initially as a CNA in home healthcare, becoming a Registered Nurse in 2012. She has worked in home health care, hospice, nursing homes, assisted living centers, rehabilitation facilities, substance abuse centers and psychiatric hospitals. For the past 4 years she has worked in Medicare as a supervisor, so has a vast knowledge of the insurance aspect of healthcare as well. She moved to Pinedale in 2024 from South Carolina with her husband and children. She spends her free time spending time with family and her 2 dogs, traveling, catching up on the latest great book or cooking. She is excited to be a part of Sublette County.
