Noah Chutz PA-C, CAQ -EM

Noah earned his degree from the University of Washington in 2018 and has training in Emergency Medicine, Urgent Care and Addiction Medicine. Noah has his Emergency Medicine CAQ through the National Commission for the Certification of PAs (NCCPA).  Prior to becoming a PA, his career path involved international development and sustainable natural resource use that focused on Latin America and forest management.  He is a Returned Peace Corp Volunteer from Mexico and never says no to a good salsa dance. He is most often found enjoying mountains, forest and surf when out of the hospital and clinic. 

Penny Jensen - Substance Use Navigator

Penny relocated from Star Valley, Wyoming, to Pinedale and proudly started at Sublette County Health in 2021. She began her career in Patient Access before transitioning to the nursing team as a Health Unit Coordinator. Penny currently serves as the Patient Access Manager, overseeing operations at the Pinedale Clinic, Marbleton Clinic, and the hospital.

Penny also works closely with Noah Chutz, PA-C, as a Substance Use Navigator, supporting patients through some of their most challenging moments with compassion and care.

In 2025, she earned a Certificate of Completion as a Community Health Worker through the University of Wyoming, further strengthening her commitment to patient-centered care.

Penny is passionate about helping patients when they need it the most. Outside of work, you’ll usually find her cheering on her two children as she follows them around pursuing their dreams.
